<p class="">Roc is  taking a Urolithin A supplement.  Nutrition Investigator now takes 500 mg in the morning. The supplement is a pomegranite extract. Dietary sources of urolithin A include pomegranate, strawberries, blackberries, walnuts, chestnuts, pistachios, and brewed tea, which contain ellagic acid and/or ellagitannins. <p class="">Roc is considering taking Urolithin A supplements. <a href="https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2788244">These findings</a> indicate that Urolithin A promotes joint mitochondrial health, alleviates OA pathology, and supports Urolithin A&#8217;s potential to improve mobility with beneficial effects on structural damage in joints.  Urolithin A improves mitochondrial health, reduces cartilage degeneration, and alleviates pain in osteoarthritis. Urolithin A (UA) is a gut microbiome-derived natural compound that only 40% of people can naturally convert from dietary precursors at meaningful levels. Positive effects of direct UA administration in health, aging, and age-related conditions have been identified in several recent studies. No statistical differences in adverse events were observed between the 2 groups. Conclusions and Relevance This randomized clinical trial found that urolithin A supplementation was safe and well tolerated in the assessed population.</p>
<p class="">Roc is considering no longer taking leucine. Someone in GRG thinks one should not supplement leucine or BCAAs.  It&#8217;s more important to try to minimize the aging of the body and the muscles by maintaining low average mTOR activation for decades than it is to try to build a lot of muscle to prevent sarcopenia at the potential cost of accelerating systemic aging of the body. There will be more powerful options such as gene therapies and more to improve health. However these therapies will be harder to create and more needed for fixing the damages resulting from overactive mTOR than they will be for increasing muscle mass to prevent sarcopenia. Note that sarcopenia is a much easier problem to solve later on in life than reversing the damages resulting from excessive mTOR activation because increasing muscle growth in an old person is far more simple than fixing cellular damages caused by excessive mTOR. Not to mention that the latter harm will be seen in most parts of the body not just in the skeletal muscles. The <a href="https://www.foundmyfitness.com/episodes/leucine-mtor-muscle-mass">opposite point of view</a> is promoted by Peter, who  does a nice explanation of how leucine is the most important trigger and talks about how it is short lived. High <strong><em>leucine</em></strong> foods include cheese, soybeans, <strong><em>beef</em></strong>, <strong><em>chicken</em></strong>, pork, nuts, seeds, <strong><em>fish</em></strong>, seafood, <strong><em>and</em></strong> beans.  Roc’s initial conclusion: eat fish, nuts, beans, cheese.</p>

<p class="">Pregnancy for my doctor – I have a wonderful nurse who told me one of my doctors in getting pregnant.  So I compiled some of the most important Science articles I have read about how to be pregnant and improve the life of your child.  I do not include the literature research I did to increase my chance of having a son from 50% to 90%. But it did succeed!</p>
<p class="">I <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3835452/">love dark chocolate</a>. The Kuna Indians who reside in an archipelago on the Caribbean Coast of Panama have very low blood pressure levels, live longer than other Panamanians, and have a reduced frequency of myocardial infarction, stroke, diabetes mellitus, and cancer &#8212; at least on their death certificates. One outstanding feature of their diet includes a very high intake of flavanol-rich cocoa. Flavonoids in cocoa activate nitric oxide synthesis in healthy humans. <a href="https://email.nutrition.org/c/1KsKlJdrVYvBF7thDDwSL4S0bSPy" target="_blank" rel="noopener"><em>Effect of cocoa flavanol supplementation for the prevention of cardiovascular disease events: the COcoa Supplement and Multivitamin Outcomes Study (COSMOS) randomized clinical trial.</em></a> Cocoa extract supplementation did not significantly reduce total cardiovascular events among older adults but reduced CVD death by 27%.</p>

<p class="">Science 27 May Focus: Microbiome: pg 932 “The bulk of the microbiome is in the gut, but distinct microbial communities exist on the skin and in the mouth, nose, lungs, and genital tract. The human microbiome has important roles in maintaining homeostasis, and disruption of microbial colonization of an infant has systemic effects that may influence health later in life, potentially promoting the development of autoimmunity, allergies, metabolic diseases, and even cancer. If a microbial population is disrupted by antibiotics or a low-fiber diet, then pathogens can gain advantage, resulting in systemic infection and inflammation.” Pg 935 – oral microbiota in teeth and tongue affect colorectal cancer and rheumatoid arthritis. Pg 936-Brain function requires microbiota rto control neurological disease. Amyotrphic lateral sclerosis and autism may be caused by unhealthy microbiome. Pg 943-skin microbiome regulates lung inflammation, pain, and cancer. Pg 945-Immune – microbiome interactions in early life determine health and disease long term- Natural birth and breast feeding are vital to establish initial microbiome in a child.</p>
<p class="">KEY LESSONS-Avoid antibiotics, eat plenty of fiber, eat kefir, kimchi or yogurt regularly, natural birth and breast feed.</p>
<p class="">     Why I take 3 mg of melatonin 4 times a week. There is no requirement for melatonin.  However, in my long notes, I reported that I began taking it decades ago.  I, Roc Ordman, take 3 mg of melatonin four nights a week. I learned at an AGE meeting years ago that taking melatonin turns off genes that make us old, and turns on genes that make us young, but that presentation was never published. In 2020, Advances in Nutrition reported that daily doses of melatonin over 1mg converts white adipocytes which store fat into brown adipocytes which burn fat and produce muscle.  Daily melatonin (at least 1mg up to 5) reduces obesity in post-menopausal women. In response to treatment, fat mass decreased in the melatonin group by 6·9% compared to placebo. A borderline significant increase in lean mass of 5·2% was found in the melatonin group compared to placebo. <a href="https://academic.oup.com/advances/issue/11/2">Mar 2020 Adv Nut</a>.  And all of that is now reinforced by its effects preventing/helping arthritis.  I still go with 3mg 4 times a week, so that my body continues to make melatonin naturally the other three days.  In addition to the pineal gland where it is made to promote sleep, they have now found that it is made in many other organs, likely promoting these many other benefits.</p>
<p class="">     3 mg melatonin is a common dosage for melatonin in trials.  <a href="https://pubmed.ncbi.nlm.nih.gov/24802882/">Optimal dosages for melatonin supplementation</a> therapy in older adults: a systematic review of current literature.  In older adults, we advise the use of the lowest possible dose of immediate-release formulation melatonin to best mimic the normal physiological circadian rhythm of melatonin and to avoid prolonged, supra-physiological blood levels. Pre- and post-intervention levels revealed a significant elevation of the post-intervention melatonin levels in a dose-dependent fashion.</p>
<p class=""><a href="https://www.jstage.jst.go.jp/article/jnms/70/4/70_4_334/_article">Double Blind Study of Melatonin Effects</a> on the Sleep-wake Rhythm, Cognitive and Non-cognitive Functions in Alzheimer Type Dementia used 3 mg dosage and found improvements in sleep and cognition. <a href="https://pubmed.ncbi.nlm.nih.gov/15249281/">Melatonin administration at a dose of 1 mg</a> nightly may be effective in improving certain aspects of cognitive functioning and subjective reports of sleep quality in elderly subjects.  Another study used 3 mg in healthy people and found it contributed well to healthy sleep.</p>
<p class="">Active niacin has benefits in de-myelination disorders, MS, Parkinson’s Disease, dementia &amp; Alzheimer’s. <p class="">High intensity <a href="https://www.dovepress.com/article_75543.t161609909">interval training is great</a> to prevent sarcopenia. The American College of Sports Medicine (ACSM) proposed every adult to strengthen and maintain the functions of cardiopulmonary through moderate exercise 30–60 min per day (≥5d per week), or vigorous exercise 20–60 min per day (≥3d per week), or a combination of moderate and vigorous exercise per day (≥3-5d per week). High intensity interval training (HIIT) is characterized by repeated short to long bouts of relatively high-intensity exercise (≥90%VO2max or &gt;90–95% HRmax for 6 s to 4 min) alternate with recovery periods of either low-intensity exercise or rest (ranging from 20% to 40% VO2max for 10 s to 5 min), emerged as an alternative for traditional continuous training.  Firstly, HIIT displays comprehensive effects on exercise capacity and skeletal muscle metabolism. HIIT induces great growth of muscle, prevents skeletal muscle atrophy, and improves the motor function via promoting great phosphorylation of mTOR and rps6 and inducing the expression of transcriptional coactivator peroxisome proliferator-activated receptor γ coactivator 1α (PGC-1α), which is crucial for mitochondrial biogenesis.  Sarcopenia, an age-related disease characterized by loss of muscle strength and muscle mass, has attracted the attention of medical experts due to its severe morbidity, low living quality, high expenditure of health care, and mortality. Traditionally, persistent aerobic exercise (PAE) is considered as a valid way to attenuate muscular atrophy. However, nowadays, high intensity interval training (HIIT) has emerged as a more effective and time-efficient method to replace traditional exercise modes. compared with PAE, HIIT showed similar or even higher effects in improving muscle strength, enhancing physical performances and increasing muscle mass of elder people.</p>

<p class="">Since vitamin B3, niacin, is absorbed readily, while NAD+ and NMN are expensive and likely broken down in the stomach, this research strongly supports other studies that taking niacin is important to mitigate age-related disorders.  Preclinical studies have revealed that the elevation of nicotinamide adenine dinucleotide (NAD + ) upon the administration of nicotinamide mononucleotide (NMN), <a href="https://www.nature.com/articles/s41514-022-00084-z">an NAD + precursor, can mitigate aging-related disorders</a>; however, human data on this are limited. We investigated whether the chronic oral supplementation of NMN can elevate blood NAD + levels and alter physiological dysfunctions in healthy older participants. We administered 250 mg NMN per day to aged men for 6 or 12 weeks in a placebo-controlled, randomized, double-blind, parallel-group trial. Chronic NMN supplementation was well tolerated and caused no significant deleterious effect. Metabolomic analysis of whole blood samples demonstrated that oral NMN supplementation significantly increased the NAD + and NAD + metabolite concentrations. There were nominally significant improvements in gait speed and performance in the left grip test, which should be validated in larger studies; however,<br />
NMN exerted no significant effect on body composition. Therefore, chronic oral NMN supplementation can be an efficient NAD + booster for preventing aging-related muscle dysfunctions in humans.</p>

<p class="">If you are a caregiver for someone living with dementia, it can be helpful to maintain a consistent daily routine and bedtime, help the person minimize or avoid caffeine, nicotine, and alcohol, and pay attention to “triggers” in the environment that may cause the person to become anxious or agitated. If the person is restless, a brief walk can be calming.</p>
<p class="">Consistency is key to a good night’s sleep: make it a habit to wake up at the same time each morning and go to bed at the same time each night to help your body adjust to a regular sleep schedule. If you cannot fall asleep after 20 minutes in bed, get out of bed and perform a relaxing activity with low light, like stretching or reading.</p>

<p class="">I have decided to stop taking aspirin regularly that was once thought good for one’s heart- From Kevin Fickenscher: The regular consumption of aspirin – either regular or baby-aspirin offers “…little benefit for most healthy people, and…it may contribute to a risk of bleeding in your stomach or brain that goes up as you get older.” From  U.S. Preventive Service Task Force</p>
<p class="">From Kevin Fickenscher: 18% of healthcare workers have quit their jobs since the start of the pandemic.  In fact, 20% of physicians and 40% of nurses intend to leave practice completely.</p>
